The Achilles tendon attaches the calf muscle to the heel bone. Overuse or injury can cause the tendon to become chronically inflamed, resulting in pain and decreased mobility. Over time, scar tissue can accumulate on the tendon.

How to Prepare for the TOPAZ® Procedure for Achilles Tendinosis

Your Resurgens physician will review your pre-operative checklist with you. These precautions for Achilles tendinosis surgery include:

  • Stopping the use of anti-inflammatory medications (NSAIDs) three days prior to the procedure. This includes ibuprofen, naproxen, and other NSAIDs.
  • Stopping the use of any steroid-based medication, such as cortisone or medrol, for at least two weeks prior to the procedure.
  • In some cases, the use of blood thinners, such as aspirin, Plavix, or Coumadin, should be suspended at least seven days prior to the procedure. Your Resurgens physician will determine if this is necessary, and you must get approval from your cardiologist and/or primary care physician before stopping any blood thinning medication.
  • Arranging for transportation to and from your procedure, as the light sedation used will affect your ability to drive.

What Happens During the TOPAZ® Procedure for Achilles Tendinosis?

Preparation - In preparation for Achilles tendinosis surgery, the patient is positioned and local anesthesia is administered. The surgeon draws a grid pattern on the back of the ankle, indicating the locations that will be treated.

Coblation - At each of these locations, the surgeon carefully inserts the TOPAZ® device, which resembles a specialized sewing needle, through the skin and guides it down to the Achilles tendon. When activated, the device emits radio frequency waves that dissolve inflamed scar tissue within the tendon. By dissolving this thickened tissue, the surgeon strategically creates a new injury on the tendon that brings blood flow back to the area and will heal under controlled circumstances.

End of Procedure - When the procedure is complete, the ankle is bandaged. Some patients may be allowed to bear weight on the foot immediately, others may be placed in a cast or splint for two to four weeks.
